Please complete the form below in its entirety. After your application has been processed, we will reach out to you via your Western email to schedule an Access Planning Meeting. During this meeting, an accommodation manager will seek to understand your access barriers.

Please know that the information you provide will be kept private in accordance with the Family Education Rights & Privacy Act (FERPA).

If you are already registered with our office and you want to update your accommodations, please don't fill out a new application. Instead, please call 360-650-3083 or email drsfrontdesk@wwu.edu to schedule an appointment with your accommodations manager.
